Helen’s Trust works in partnership with Blythe House Hospicecare to provide 24/7 hospice care in the comfort of patient’s homes across the High Peak, Derbyshire Dales and North East Derbyshire.
Referrals are made to the team for a variety of reasons including respite sits so that a carer can get some much-needed rest or to run errands; to get a patient home from the hospital; crisis avoidance, where a family is in need of support quickly to help with a potentially distressing or emotional situation, and for end of life care when a patient is in the final weeks or days of life.
Ashgate Hospice provides care to patients across North Derbyshire at the hospice and in the community.
For patients with the most complex of needs, they have an Inpatient Unit. However, most of their patients are cared for at home, where a team of specialist nurses and healthcare professionals enable them to stay in familiar surroundings for as long as possible.
Blythe House Hospicecare provides free services to patients and families in the High Peak and surrounding communities who are affected by life-limiting illnesses including cancer, heart failure, chronic obstructive pulmonary disease (COPD), and neurological conditions such as motor neurone disease.
The hospice also supports those who are bereaved. The 24/7 Hospice at Home service provides palliative and end of life care to patients who wish to be supported in the comfort of their own home.
Since April 2020, Blythe House has worked in partnership with Helen’s Trust to provide more care to patients across the High Peak, Derbyshire Dales and North East Derbyshire.
